CAPS Form Requirements

Important: Per WGTC policy, enrollment verification requests cannot be received or completed until the 4th day of classes, after the semester's Drop/Add period has ended.

Please allow up to 3 business days for processing after all required information has been received.

Please complete all of the following steps:

  1. Obtain the current CAPS form from your caseworker.
    • The form must state "Effective Date: October 6, 2025" at the bottom.
  2. Before submitting:
    • Ensure all required fields on page 1 are filled out.
    • Include your signature and date.
  3. Email Registrar@westgatech.edu pages 1 and 2 with the first page completed
  4. Send the email from your WGTC student email account.
  5. Include your Student ID number in the body of your email.

Note: Processing will begin only after the 4th day of classes and once all required information has been received. Please allow up to 3 business days for your completed form to be returned.
